Lab instructors are required to educate themselves and their students about laboratory safety.
These forms are fillable:
- Accident Reporting Form; (undergraduates in a teaching laboratory) - send this form to Dawn Stoddard, the lab coordinator for your course, and to the ISU risk manager, Luke Jepma.
- Workers' Compensation (graduate students & employees)
