Coordinate continuity of patient care with external healthcare organizations and facilities, including referrals to specialty care providers, and continuity of patient care with patients and families following hospital admission, discharge, and ER visits. Promote clear communication among the care team and treating clinicians by ensuring awareness regarding patient care plans.

Urgent care and appointment physicians work together to address the patient's episodic and chronic care problems. Clinics provide urgent care providers the resources and staff to efficiently manage acute care problems. All our clinics are Patient Centered Medical Homes.
