Religious Accommodations for Vaccine Exemption

Idaho State University is required to reasonably accommodate sincerely held religious beliefs which conflict with an academic requirement, unless providing the accommodation would create an undue hardship to the University. 

In order to be considered for a religious exemption for vaccination, choose the appropriate form from the list below. If you are unsure which form to use, please email us. 

Note: Accommodations for vaccination exemptions that are not based on religion should be submitted to Disability Services.
