Practicum: Agency and Field Instructor Information

Agencies interested in becoming an approved Field Practicum Agency Site for the ISU Social Work Program may complete a Practicum Agency Profile and submit this application to the Field Director.

Potential Field Instructors within the agency may complete the Field Instructor Background Form and submit it as well.

Upon notification of approval, the agency and its field instructors will receive login information for the Intern Placement Tracking (IPT system). This web-based system allows agencies to post current information regarding practicum placements and enables students to view prospective field agency placement sites.
