Instructions for Alternative Testing through Disability Services, DS
Pocatello, Idaho Falls, and Meridian courses
Student responsibilities:
Step 1) Schedule Exam
Reminder of your responsibility:
You, the student, are responsible for following the steps below to schedule your upcoming exam with a Disability Services, DS, testing center. Please review the following scheduling deadlines.
- You must schedule your exam at least 48 business hours before the start of the upcoming exam.
- You must schedule final exams by Monday, April 27th.
If you do not scheduled your test through a DS testing center by the deadlines mentioned above, then you will take the exam along with the rest of the class where none of your alternative testing accommodations have to be honored.
For steps on how to complete this responsibility, the student can review the information below:
Step 1) Go to the Services Portal.
Step 2) Sign in to the Services Portal using your username and password.
Step 3) On the portal dashboard, go to the My Accommodations section and click on the Alternative Testing option.
Step 4) Within the Alternative Testing Agreement(s) section, select the class you are scheduling an exam for by clicking on the drop-down box next to the Select Class.
Step 5) Click on the Schedule an Exam button.
Step 6) Review the TERMS AND CONDITIONS OF SCHEDULING EXAM section on your screen.
Step 7) Scroll down this page and Fill out the fields in the Exam Detail section.
Step 8) Click the Add Exam Request button at the bottom of the Exam Detail section.
Step 9) A green check mark will then appear at the top of the page to confirm that you have successfully submitted your exam request.
Step 10) Now that you have scheduled your test, please check your email account regularly for any updates about your exam request.

Step 2) Review Testing Agreement for Exam
For steps on how to complete this responsibility, the student can review the information below:
REVIEWING THE TESTING AGREEMENT VIDEO
Step 1) Go to the Services Portal.
Step 2) Sign in to the Services Portal using your username and password.
Step 3) On your dashboard, go to the My Accommodations section and click on the Alternative Testing option.
Step 4) Within the Alternative Testing Agreement(s) section, select the class related to the DS Testing Agreement you would like to review by clicking on the drop-down box next to the Select Class:
Step 5) Click on the View Alternative Testing Detail button.
Step 6) Review the DS Testing Agreement
Step 7) If you have questions or concerns with the DS Testing Agreement, then please contact your instructor to address questions or concerns.
For example, you may have been been under the impression that your upcoming test was open book. However, the Open Book option is not selected within the allowable materials question of the DS Testing Agreement. You would contact your instructor to discuss this misunderstanding. If open book is allowed, then your instructor will contact our office to make that change on the DS Testing Agreement.
Step 8) Now that you have reviewed the DS Testing agreement for your upcoming exam, please check your email account regularly for any updates about your exam.

Instructor responsibilities:
Step 1) Fill out the DS Testing Agreement for the course
Fill out the DS Testing Agreement that is listed at the beginning of the accommodation letter. For steps on how to fill out this DS Testing Agreement, the instructor can review the information below:
FILLING OUT THE TESTING AGREEMENT VIDEO
Step 1) Within your email account, open the Letter of Accommodation email that was sent to you. The Letter of Accommodation email starts with the characters [DS], followed by your student’s name.
Step 2) Review this email and carefully read through the Alternative Testing Information section.
Step 3) In the Alternative Testing Information section, click on the DS Testing Agreement link. Note: Even if your course only has online exams and/or quizzes, still follow this step. The DS Testing Agreement will allow a student who may want to use a DS testing center to use testing accommodations (i.e. a reduced distraction room) to test with parameters you establish. Even if you plan on proctoring these exams for your student or your course has no exams, still follow this step. The next step, Step 4), will allow you to communicate this situation to DS.
Step 4) Review the two option from the drop down list within the PROCTORING YOUR OWN EXAM.
A) If you are proctoring your own exams for this student or your course does not have exams, then select an option from the drop down list and select the option that applies to your situation and then click on the Confirm button. Note:There are no other steps you need to complete since you will not be needing to fill out a DS Testing Agreement. You can close out of the DS Testing Agreement page you are on.
B) If your testing situation is not satisfied through either of these options, then skip this section. Proceed to the Alternative Testing Agreement section and answer each question.
Step 5) Once you complete the DS Testing Agreement questions section click the Submit DS Testing Agreement button at the bottom of the page.
Step 6) A green check mark will then appear to confirm that you have successfully submitted the DS testing agreement.
Important note: Instead of receiveing this green check mark, you may be taken to the online services sign-in page with a notice that the session has expired.
If this occurs, then click the back button on your web browser.
You will then see the green check mark that confirms your agreement has been submitted.
Step 7) You will then receive an email containing the testing parameters you established. Contact the DS office on your campus if you need to change the parameters.
Step 2) Provide Exams to DS Testing Center
General Reminder:
You, the instructor, are only responsible for providing an exam to the appropriate Disability Services, DS, testing center if the student has scheduled their exam through a DS testing center. You will be notified if the student has scheduled an exam with a DS office via an email to your ISU email. This email will come from our office and the subject of this email will begins with the phrase, [DS] Student Testing Request Approved. If the student has not scheduled their exam through a DS testig center, then the student will be taking the exam along with the rest of the class where none of their alternative tetsing accommodations have to be honored.
PROVIDING EXAMS TO TESTING CENTER VIDEO
Methods of delivery for exams:
You can deliver exams and/or exam passwords by:
- Uploading through the DS Testing Upload Link
- Emailing to ada@isu.edu
- Dropping off to the applicable DS office
- Faxing to the applicable DS office
Steps to Uploading through the DS Testing Upload Link:
Step 1) Once your student schedules their exam and it is approved by DS, then an email will be sent out. The subject of this email is [DS] Student Testing Request Approved and Link to Upload Exam. Open this email and review its content.
Step 2) Click on the link in the bottom half of this email.
Step 3) This link will take you to the SUBMIT EXAM ELECTRONICALLY AND EXAM INSTRUCTION page. Fill out the fields within the File Information section.
Step 4) Click the Upload Exam button at the end of the File Information section.
Step 5) A green check mark will then appear to confirm that you have successfully uploaded the exam.
Step 6) Now that you have uploaded the exam, please check your email account regularly for any updates about this uploaded exam or about the exam that your student scheduled and will be taking through a DS Testing Center.

Step 3) Additional responsibility for courses with testing done online
If the student has an extra time accommodation, then the instructor will ensure additional time is set within the learning management system being used.
