1. Please return this roster sheet to the Campus Recreation Office in Reed Gym no later than 5 PM on the end registration date.  Any rosters turned in after the specified date and time will be charged a $5.00 late fee per day! 

2. The captains meeting will be held the day after registration ends at 6:00 pm in room 309. (Exception: Floor Hockey will be held at 6:30 pm).   If the captain or a member of the team is not present at this meeting the team will forfeit playoff privileges.

3. All rosters that are turned into the Campus Recreation office are not finalized until two weeks after the season starts. Captains will have 2 weeks from the start date of the sports season to add individuals to their roster. Captains are responsible to check their rosters to make sure the players they want are on the permanent roster.

4. The roster may not contain more than 4 varsity athletes, if it does contain more than 2 varsity athletes, you must participate in a division higher than the lowest offered.

5. Co-Rec rosters must consist of an even number of men and women.

6. When submitting the roster, you must pay the registration fee.

7. The Intramural Department has a new forfeit agreement please see the attachment.

8. The Intramural Department reserves the right to edit any team name that does not deem appropriate.
