Office of Equity and Inclusion Announces Change to Forms Processes

The Office of Equity and Inclusion has converted the downloadable forms on its website to DocuSign to increase efficiency and ease operations.This includes the Search Summary form (formally the AA 3-75 form), the Emergency Hire Request, the Search Waiver Request, and the Position Description form.
The Position Description is now part of the Emergency Hire and Search Waiver Request forms, so there is no need to submit that form separately as required in the past. Once a form is completely approved in DocuSign, you may move forward in working with Human Resources to compose an offer letter and PR. You will no longer receive an approval email from the Office of Equity and Inclusion.
