CHW Apprenticeship Program: Application Instructions

Thank you for your interest in completing a registered Community Health Worker (CHW) Apprenticeship in Idaho. This application helps us determine eligibility and begin the process of apprenticeship placement with an employer.

Steps to Apply
  1. Complete the application through the link box below.
    • In the application, you will select if you have a sponsoring organization or are in need of assistance with finding a sponsor organization in your area. You will also provide information that will help determine your eligibility and best placement options.
  2. Once submitted, ISU or a partner representative will contact you about next steps, whether that be identifying a placement site or finalizing placement at a current employer.

CHW APPRENTICESHIP APPLICATION


ELIGIBILITY for CHW Apprenticeship

  • 18+ years of age
  • Employed or able to be employed
  • Have completed OR be concurrently enrolled in a CHW Core and/or CHW Advanced Training courses
  • For the duration of the apprenticeship, you must be employed in a role that can meet the CHW competency requirements of the apprenticeship. If you are unsure of whether your position qualifies for a CHW apprenticeship, please reach out to us regarding position eligibility at chw@isu.edu.
  • As an apprentice, you will be required to demonstrate and track specific CHW Apprenticeship skills and competencies

While completing the application, if your selections fall outside of this criteria, it will conclude your application. If you believe it to be in error, or would like us to review it manually, please contact chw@isu.edu.
