Student Request for Services

Students or program participants who are new to our office should follow these steps to request accommodations

Step 1) Fill out the request for services form below and provide any documentation.

Step 2) Schedule appointment to meet with a Disability Services counselor. If you haven’t submitted documentation, bring it with you to the appointment.

Step 3) Meet with Disability Services counselor to review documentation and determine reasonable accommodations.

Step 4) Once reasonable accommodations are established for you then accommodation letters will be sent to the appropriate faculty and staff to ensure equal access is provided.

Step 5) Work with your Disability Services Specialist throughout the semester to maintain equal access here at ISU.
