New Patient Registration Form

Please email a copy or photo of insurance card and/or driver's license to the firstname.lastname@example.org (encrypted e-mail) or you will need to bring your driver's license and insurance cards with you for scanning at your first appointment. Scheduling an Appointment
Appointments are scheduled by contacting the clinic receptionist at (208) 282-3282
Multiple appointments are necessary for completion of treatment, and patients work with the same student each appointment. A variety of appointment times are available, but the average appointment lasts 2 to 3 hours.
Payment and Insurance
Payment of fees is required at each appointment. We accept Cash, Visa, MasterCard, Checks, and Dental Insurance. The Dental Hygiene Clinic will submit dental insurance claims upon request and any balances not paid by the insurance company will be due upon receipt including deductibles. Please be prepared to pay any and all fees at the time of service (minus estimated insurance coverages). Due to the high amounts of bills sent to collections, these clinics must be PAY AS YOU GO.

The Dental Hygiene Clinic is located directly across from the campus library on Martin Luther King Way.

Parking is available in the parking lot directly in front of the Administration Building located on 8th Avenue, please check in with parking attendants for a parking pass or you will be ticketed. Disabled parking is available directly in front of the campus library and adjacent to the dental hygiene clinic.
The Pocatello Urban Transit buses stop in front of the clinic and across the street at the ISU Library on an hourly basis.
If you need to cancel your appointment, please contact the clinic within 24 hours of the scheduled appointment time. If two or more confirmed appointments are missed, the clinic will be permitted to discontinue your care and refer you to a private dental office.

The Teaching Environment
This clinic is a teaching and learning environment and additional time may be required for the completion of care. All treatment is closely supervised by licensed dental hygiene faculty and supervising dentists. Therefore, fees for services provided are dramatically reduced.
The students and faculty will always strive to provide dental hygiene care in a considerate and respectful manner while providing you with the most current standards of care. The ISU Dental Hygiene Clinic uses strict infection control practices as recommended by the Centers for Disease Control and Occupational Safety and Health Administration.
Patient Treatment Rights
After an assessment, an explanation of recommended treatment, alternatives, expected outcomes, risks of no treatment, and cost of treatment has been explained, consent for care will be requested. You have the right to refuse treatment at any time. When treatment is completed, a referral to a dentist will be made because we cannot legally perform all dental care needed [e.g.: diagnostic exam for restorative work (fillings), root canal therapy, periodontal (gum) surgery, etc.].
Our clinic is committed to protecting the privacy of its patients’ personal and health information. As part of our services, each patient will complete a comprehensive health history prior to beginning treatment. A copy of your records can be transferred to your dentist’s office by request, including X-rays and a Notice of Privacy Practices will be provided at your initial appointment. These practices are incorporated to protect you, the patient, as well as the student dental hygienist, faculty, and Idaho State University.
