This is an example of a laboratory requisition form that has been adapted for the online learning environment. The requisition is delivered through ISU's learning management system, MOODLE, in an activity called a Forum. From within the forum both students and teachers can ask questions and post comments and answers. There is an asynchronous (online) dialog between the teacher and the student to move through the clinical processes.

Online students are asked to review the requisition and report any irregularities. They are asked to think about what pathogens (and normal flora) they would expect to see, and what they would look like on each of the media.

After commenting on the requisition students are asked to examine the direct exam image. The image is representative of the direct exam that would normally be seen in the clinical lab. It is also linked to a slideshow to illustrate the different magnification levels and slide fields for the direct exam. After reviewing the images students are to report the direct exam and ask any questions for clarification.

Next, students are to examine the image and slideshow for the culture and to report a preliminary result for the culture. The culture image and slideshow are also representative of the culture that would normally be seen in the clinical lab. Students should include requests for any followup tests in their report. They should also explain what steps they would take after reviewing the exam results. If the organism requires a sensitivity, they will also need to include that. Students also need to keep in mind that some of the cultures may be critical values or reportables.

Students are to post their final report to the MOODLE Forum.
