Speech
Perception Test - concerned that a child's error (or errors) may be
at least partly based in problems hearing the difference between what
they say and what they're supposed to be saying. Here's a form
(with instructions) that is a slighltly modified version of the form
developed by John Locke for
his now famous Speech Production - Perception Task that should help.

Listening Lists
