General:

Welcome to the ISU student insurance web site. ISU student insurance is developed especially for Idaho State University and their eligible dependents. The Student Health Insurance Plan (SHIP) is underwritten by Ace American Insurance Company and offered by Academic Health Plans. This web site is designed to give you detailed information about SHIP and its benefits. Bookmark this site and follow the guidelines carefully to ensure that you get the most out of your health insurance benefits.

Eligibility/Enrollment:

Effective fall semester 2003, Idaho State Board of Education policy requires full-fee paying students attending classes in Idaho to maintain adequate health insurance.  All full-time domestic undergraduate students taking twelve (12) or more credit hours, graduate students taking (9) or more credit hours and applied tech session students taking 4 or more credits, who by current fee structure or by contractual agreement are required to pay full-time fees, are automatically enrolled in the student health insurance plan.  A student must show proof of comparable coverage by September 8, 2009 in order to waive coverage and have the student insurance premium removed from their bill. 


All domestic undergraduate students taking ten (10) or eleven (11) credit hours are eligible to enroll in the student health insurance plan on a voluntary basis for an additional premium.  Eligible students must complete an enrollment form and submit to the insurance company prior to September 8, 2009.

All international students taking one (1) or more credits hours  are automatically enrolled in the student health insurance plan.


All other part time students are not eligible to enroll in the SHIP

Waivers:

Students who provide proof of continuous enrollment in an alternative U.S.-based health insurance plan with comparable benefits are able to waive out of their SHIP coverage each academic year.  Before waiving the ASISU Student Health Insurance plan, please review your health insurance company to ensure that you have adequate coverage.

1.  Medical benefits of at least $50,000 per accident or illness (to include maternity coverage for policy holders);

2.  A deductible  not to exceed $2,000.00 per accident or illness;

3. co-payment for in-network providers not greater than 25 percent with the insurance paying at least 75 percent of the charges;

4. insurance company that is owned, operated and based in the United States in compliance with applicable federal laws; and

5. health insurance in force and maintained for the full academic year.

If you are covered under another plan and wish to waive the ISU student health insurance, you can access BengalWeb and click on Financial.      International students are not allowed to waive the health insurance through BengalWeb.  You must waive the health insurance prior to paying fees. Your insurance must be effective the date that school starts and cannot be voluntarily canceled and enrollment must be continuous through the current academic year. By waiving the insurance on your fall billing, you will not be eligible for insurance coverage in the spring unless extenuating circumstances exist. Exceptions will be made if the student has a documented change in insurance status such as change in marital status, termination of dependent coverage because of attained age or change in job status causes a student to lose health insurance coverage.

Policy Period Coverage Dates For School Year 2009/20010:

Coverage for all insured students becomes effective for Fall semester at 12:01 a.m., August 15, 2009 and will terminate at 12:01 a.m. January 4. 2009. Coverage for Spring semester will become effective at 12:01 a.m. on January 4, 2009 and will terminate at 12:01 a.m. August 15, 2010. Coverage for students and dependents becomes effective on that date or the date application and full premium are received by the Company (or its authorized representative) whichever is later.

Optional Major Medical $100,000 coverage is available for additional premium for full time students only.  Contact ISU Student Health Insurance office for additional premium amunts.

Refunds:

All eligible students are enrolled in the insurance program in the Fall of each year or at the time of initial enrollment at ISU. If a student does not waive the insurance before fees are paid and has comparable health insurance, the student may request a refund of the premium by signing a refund petition (see insurance refund petition form). A refund petition must be submitted on or before insurance refund deadline  If a student withdraws from school before the first 31 days of school, the student is eligible to apply for an insurance refund.  Refund checks are not processed until four weeks after completion of refund petition or until at least three weeks after the actual date of payment for the semester. All student health insurance refunds are paid by the Student Health Insurance Carrier and are mailed to the address listed on the student health insurance refund petition. If the student has an outstanding ISU fee loan, the insurance premium will be applied to the ISU fee loan rather than being sent to the student.

Refund Appeals:

Contact the Student Health Insurance Coordinator for information on the student insurance appeals process. All student health insurance refund appeals should be submitted in writing before the end of the semester for which the student is appealing.
