Authorized Signatures Form

The Authorized Signatures Form provides the ASISU Office with the names of those permitted to authorize money transactions for your student organization. To request money from your ASISU club account, you will need two (2) signatures. It is suggested you have at least four (4) accountable people on this form and that one of the signers be the group’s advisor. You can get this form from the ASISU office, located in the hypostyle of the Student Union Building.
