Outside Contractor Information Form

By checking this box, you acknowledge that the contractor is aware that building and components are under warranty. If anything is done to void warranty, the contractor doing the work will honor warranty period and repair existing damaged work.

Work must be inspected and approved by the Division of Building Safety. Contact person will certify that the work is complete and sign DBS forms. ISU has the right to accept or reject work not in compliance with existing building. Closeout procedures include Electronic (AutoCadd) as-built drawings and electronic specifications to ISU.
