and 4th Annual Thomas Geriatric Health Symposium

"Transitions of Care"

Thanks to the generosity of an anonymous donor, a no-cost, interdisciplinary geriatric health symposium is held annually for students and clinicians in the rehabilitation sciences. The first symposium was held in December 2008 and focused on mental health. The second symposium took place on October 30, 2009 and the topic was "Balance and Fall Prevention." The 2010 symposium, held on October 29 focused on "Collaborative Care for Chronic Disease." This year's symposium's overall theme is "Transitions of Care."

Session 1, Keynote: LEADERSHIP IN A SHIFTING HEALTH CARE SYSTEM - Career Transitions for Clinical Professionals

Keynote Speaker: Jeri W. Dunkin, PhD, RN, CHCE, FAAN

Presentation Description:
Drawing from the 2010 Institute of Medicine report "The Future of Nursing", Dr. Dunkin will discuss the importance for nurses and allied health professionals to transition their own careers via education and increased scopes of work in order become full partners with physicians in improving health care quality.

Presenter's Biographical Sketch:
Dr. Dunkin is a member in good standing in the Alabama State Nurses Association. Her many contributions to the improvement of nursing - at the national and international levels - flow from her passion for helping rural nurses and the people in their care. Early in her career she focused on recruitment and retention of nurses in rural hospitals, skilled nursing facilities and community health. She joined the Rural Nurse Organization, which provides a support system for all nurses working in rural areas and still serves as its executive director today. Among other accomplishments, she established the Online Journal of Rural Nursing and Health Care to provide rural health professionals with an open access journal that focuses on rural health care issues.

Session 2: FROM HOSPITAL TO HOME - Best Practices from the Idaho Person-Centered Hospital Discharge Model Grant

Presenters: Debra Aubrey, Ph.D., and Russell Spearman

Presenters' Biographical Sketches:
Dr. Debra Larsen Aubrey is a clinical psychologist and a Research Associate Professor with the Institute of Rural Health at Idaho State University. She currently is President of the Idaho Psychological Association and is an invited peer reviewer for multiple journals and national grant agencies. Dr. Aubrey received consecutive NIH Clinical Disadvantaged Researcher LRP awards for her research in rural health issues.

Russell C. Spearman is a Senior Research Associate with the Institute of Rural Health at Idaho State University. Prior to this Russ was responsible for developing and implementing all aspects related to Idaho's 1915 C Medicaid Home and Community Based Services Waiver for adults with a traumatic brain injury. He is the former Executive Director for Idaho's Governor's Council on Developmental Disabilities.

Session 3: THE RIGHT PERSON IN THE RIGHT PLACE - Rehabilitation Criteria for Discharge Placement Recommendations

Presenter: Cindy Seiger, PT, Ph.D.

Presenter's Biographical Sketch:
Cindy started working at Idaho State University in 2006 as a full-time Assistant Clinical Professor & Clinic Director in the Department of Physical & Occupational Therapy. She co-ordinates the on-campus Physical & Occupational Therapy Clinics, which provide rehabilitation for orthopedic conditions, vestibular evaluation and rehabilitation, and many other specialties. She received her Ph.D. in Physical Medicine and Rehabilitation from Brigham Young University.

Session 4: FALLING THROUGH THE CRACKS - The Value of an Interdisciplinary Transition of Care Clinic After Hospitalization

Presenters: Eliza Borzadek, Pharm.D., and Diana Krawtz, NP

Presenters' Biographical Sketches:
Diana B. Krawtz, MSN, ARNP graduated from the University of Akron in 1980 with her BSN. She worked for 10 years at the Cleveland Clinic Foundation where she eventually became the supervisor of their home peritoneal dialysis unit. She obtained her master's degree and training for a nurse practitioner license from the University of Florida in 1997. The majority of her experience is in the chronic care of nephrology patients. The last five years she has been involved with the Transition of Care clinic within the ISU Family Medicine Residency Program, caring for patients recently discharged from the hospital in the clinic, skilled nursing facilities, and patient homes.

Eliza Borzadek, RN, Pharm.D., BCPS graduated from Idaho State University in 2004 with her Doctor of Pharmacy degree. She completed a one-year Pharmacotherapy Residency in 2006 at Pocatello Family Medicine and is board certified in Pharmacotherapy. She is a clinical assistant professor at Idaho State University, Departments of Pharmacy Practice and Family Medicine where she serves as a preceptor for pharmacy students on medicine clerkship. She also provides drug-specific recommendations during in-patient medicine rounds at Portneuf Medical Center. For the last five years she has been involved in medication reconciliation activities at the hospital and in the Transition of Care clinic at Pocatello Family Medicine.
