Non-Cash Gift Acceptance Form

Please use the Non-Cash Gift Acceptance Form to donate a non-cash gift. You will complete the top portion, print it out and include it with your item(s) to be donated. Please deliver them to the specific department or organization accepting the gift. The department will complete their section and forward the form to the Foundation. The Foundation will then issue you a receipt for tax purposes.
