Parent Plus Loan Forms - choose appropriate year
- Complete the application for the Plus Loan and return it to ISU Financial Aid Office.
- After you have been approved for a Plus Loan you will need to complete the PLUS Electronic Master Promissory Note (MPN) using your FAFSA Personal Identification Number (PIN).

If you would like to apply for a Federal Graduate PLUS Loan, three items need to be completed.
- Graduate PLUS Loan Pre-Approval (form below)
The Graduate PLUS Loan Application process will access your credit history to determine whether your loan will be approved or denied. If the loan is approved, follow the online instructions to complete the Entrance Counseling and Master Promissory Note.
- Graduate PLUS Entrance Counseling (New link not available yet.)
This requirement applies only to first time Graduate PLUS borrowers for PLUS loans disbursed after July 1, 2008. If you have previously borrowed a Graduate PLUS loan at ISU, you do not need to complete entrance counseling again.
- Graduate PLUS Master Promissory Note (MPN) required for the first time Graduate PLUS Loan borrowers at ISU, is the legal document you must sign agreeing to pay back you loan. You may now complete and sign a Federal Graduate PLUS Loan (MPN) electronically using your federal Personal Identification Number (PIN) at the link below. To obtain a PIN, visit the U.S. Department of Education PIN site.
