Please use this form when requesting work to be billed to your department.
- If your job request is for routine maintenance, please use the Maintenance Request form.
- If your job is related to changing a space or starting a project, please use the Project Space Request form.
- If you are unsure of how to classify your request, please refer to Billable vs. Non-billable Services or call extension 2784.
- This form is for faculty and staff only. If you are a student, please contact your supervisor or fill out a Maintenance Request form.
