How to Apply
For your application to receive full consideration, all parts must be completed and submitted by the annual deadline of November 15. Follow these eight steps to completing your DPT application.
- Register with Physical Therapist Centralized Application Service (PTCAS).
- Submit your application for graduate studies to the Idaho State University Graduate School, and pay $55 Graduate School application fee by November 15.
- Submit official transcripts to the ISU Graduate School and PTCAS. Transcripts must be hardcopies; official transcripts submitted digitally will not be accepted.
- Submit official Graduate Record Examination (GRE) or Miller Analogy Test (MAT) scores to the ISU Graduate School.
- Submit three letters of recommendation to PTCAS.
- Submit Physical Therapy observation verification form to PTCAS.
- Pay DPT Program Application fee of $45 by November 15 with a completed fee submission form (mail to DPT Program, 921 S 8th Ave, Stop 8045, Pocatello, ID 83209).
- Submit your completed PTCAS application by November 15.
Failure to complete all of the above by the annual deadline of November 15 may disqualify your application from being considered.
