The following are procedures for waiving health insurance for Idaho State University Fall, 2009 and Spring, 2010. All full time fee paying students will be allowed to waive the student insurance, if they provide proof of other adequate medical insurance coverage. Prior to submitting a waiver it is your responsibility to thoroughly read the Waiver standards.

International students please contact the student health insurance office for a waiver.

The waiver form may be accessed thru the following steps:

1. Log In to BengalWeb

2-Under AFinancial Information@ select ACharges/Fees/Awards/Insurance@

3-Select AInsurance Waiver@, then submit request

4-The screen will show the eligible years for waiver, select 2009-20010

5-Complete all required information on waiver form (do not use a 1 before the area code in the phone number section)

6-Select ASubmit@

Waivers can be completed until the time of payment. Changes to your bill are updated nightly.

If you do not have computer access, you may call 282-2972 and request an insurance waiver form be mailed.

Students who pay insurance and then wish to receive a refund have until January 28, 2010 to request an insurance refund in the student insurance office, located in the ISU Student Health Center. To complete an insurance refund request, students are required to provide proof of adequate health insurance.
