If you are full-fee paying student, you are automatically billed for the student health insurance program at Idaho State University. You may waive this fee if you have adequate health insurance. Your policy must remain in effect during the current academic year. International students, please contact the student health insurance office for information regarding waivers and refunds.

Q.  How do I get an insurance card?
A.  The Student Health Insurance Company will send you an insurance card with an ID number.  This will come to you in the mail.  If you need a card before you receive one, you can download a temporary card at:  https://www.academichealthplans.com/isu/2009-2010/temporaryIDCard.php.

Q. What does the policy cover?
A. Covered services are paid at 80% of the Preferred Allowance for services rendered by Preferred Providers. Services obtained by Out-of-network providers will be paid at 60%.

Q. Is there a deductible?
A. Yes. The deductible is $250/academic year.

Q. Is their coverage for maternity?
A. Yes. It is covered as any other illness at 80% of usual and customary charges for in-network providers and 60% for out-of-network providers. One ultrasound is allowed regardless of diagnoses. Medical necessity would need to be proven for additional ultrasounds.

Q. Is pre-certification necessary?
A. No.

Q. I need to have a physical done, is it covered under the policy?
A. One physical exam per year is allowed at the Student Health Center up to $ 10 maximum, their number is 282-2330. One pap smear is covered at the Student Health Center per year.

Q. It is the middle of the semester and a student wants to purchase the insurance, can they?
A. If they refunded the insurance, they may not purchase the insurance unless they are losing existing coverage. There is no discounted rate, it is still $523. The same is for spouse and children, there is no discounted rate either.

Q. Can part-time students get the insurance?
A. To meet the eligibility requirements, you must be carrying at least 12 credits for undergraduate, 9 credits for graduate students and 4 credits as an Applied Tech session student and paying fulltime fees or International student taking at least 1 credit or more.

Q. Do I have to purchase the ISU Health Insurance
A. If you are covered under another health insurance plan and wish to waive the ISU student health insurance, you can access this form through MyISU portal at www.isu.edu. Students who pay insurance and then wish to receive a refund, will follow the same procedures as previous years. You have ten days, from the first date of class to request an insurance refund in the student insurance office, located in the Student Health Building. International students are not allowed to waive the health insurance.

Q. Is there coverage for my spouse and children?
A. Yes. (Refer to price list)

Q. How do I file a Claim?
A. A company claim is required for filing a claim. Claim forms are available from the Student Health Center, by calling Academic Healthplans at (888) 308-7320 or online at www.AHPCare.com/isu. Mail to: Academic Healthplans, c/o True Choice USA, P.O. Box 700308, Dallas, Texas 75370-0307.

File claims within 30 days of injury or first treatment for a sickness or as soon as reasonable possible. Bills should be received by the Company within 90 days of service. Bills submitted after one year will not be considered for payment except in the absence of legal capacity.

Benefits to Keeping Student Insurance

If you have other insurance, this insurance will become secondary and will help with deductibles and co-payments. The insurance will be primary at the student health center .

What About Emergencies?

Emergency room visits are expensive and should be reserved for emergencies only, i.e., poisoning, heart attacks, severe injuries or burns, etc. Visits outside the Student Health Center will be billed at standard rates subject to the deductible and co-payments.

Specific Items Not Covered By the Policy

The policy DOES NOT cover pre-existing conditions. The only way a pre-existing would be covered is if the student has been continually covered under the policy for 6 months. For questions regarding portability of prior insurance coverage, please contact the insurance company directly.
