Job Name/Application: For HP3000 Servers, enter the name of the stream job, LISTOP for Hplist, or LABELS for Labels.  For NETWORK or HP9000 UNIX Servers enter the name of the application from which the output was generated. 
Job Number: For HP3000 Servers enter the following:
1) Session Number (S###) for HPLIST or LABELS, 
2) Job Number (J###) assigned to submitted job.
Note: NOT USED for NETWORK or HP9000 UNIX Servers!  However, if it's the first job, just put NA in the box to process the work order.
Form code: Make the choice applicable to the form choice that applies to this job request.

Tape-In: The input tape number to be read.  You can type SCRATCH in this space, if you need Operations to use a scratch tape. You can also type INSTRUCTIONS if you need to relay information to the Operator about where the tape can be found, if you do not know the tape number or it's not a scratch tape.
Tape-Out: The output tape number for writing. You can type SCRATCH in this space, if you need Operations to use a scratch tape. You can also type INSTRUCTIONS if you need to relay information to the Operator about where the tape can be found, if you do not know the tape number or it's not a scratch tape.
Sys-CD: The output tape number system code. 
Example: 99 - for system 99 (The system code is assigned by CC Operations if not provided.) 
Retention:
The amount of time you want the output tape to be retained. 
Example: 365 for one year retention.
Tape Description: The output tape description.
If Job Fails:
Enter the choice of what to do, should the job fail. Choices include... 
Stop And Do Not Continue: This informs the operator, that should the job fail, they must stop and if it's possible contact the person who requested the job, so the problem can be resolved.  If it's during non-business hours, they are to wait until the next day business hours, to have the person who requested the job, informed about the problem.
Continue On: This lets the operator continue to the next job on the list, if the job should fail.  There is no need to inform the person who requested the job, nor contact the analyst.
Stop And Call Analyst: This informs the operator that is the job fails, they are to contact the Analyst immediately.
Stop And Call Analyst At Regular Hours: This informs the operator that if the job fails, they are to contact the Analyst during their regular working hours.
