  1. Project Description:
    Briefly describe the project.
  2. Project Scope:
    Describe the overall objectives. List each department involved.
  3. Database Modifications:
    List any addition, deletion or modification of data sets and/or fields and what database(s) will be affected.
    Create/modify and attach a new/revised database structure chart.
    Provide descriptions for new datasets and/or fields (i.e. data dictionary).
  4. System Processes:
    Specify by name and describe the functions in each process/program to be developed.

  5. Development Plan:
  6. Indicate the proposed order of processes/programs to be developed.
    Specify any conversion plans.
    Identify projected hours for each step.

  7. Testing Plan:
    Indicate the steps for testing processes/programs and the parties involved.
    Identify projected hours for each step.
  8. Implementation Plan:
    Indicate the steps for implementation and the parties involved.
    Identify projected hours for each step.
