Application for Accommodations Forms

All students applying for services at the ADA center are required to fill out the following forms. It is recommended that you bring a completed copy of the application with you to your first meeting.

The testing accomodation form is REQUIRED for each test taken at the center. A seperate form is needed for each test. You may print the form here.

Barrier Removal Form

If you have encountered an barrier to access at ISU, such as an inoperative elevator or lift, blocking of an access ramp, broken or missing curb cut, or other impediment to access, please let us know. You may use this form or call at 282-3599.

Alternate Chairs or Tables in Classrooms

If have specific needs regarding tables in chairs in your class room, you can submit a request by emailing or calling the ADA Center. Specific policy information can be found here.

Ad Hoc Interpreter Request Form

If you would like to attend an activity that is not on your original class schedule and want to have service providers, you need to fill out an interpreter request form located in the ADA office. These need to be filled out at least three working days in advance to provide enough time to secure service providers. If you find out less than three working days prior to the event and would still like to attend, you can still fill out the forms and the ADA Director will still attempt to find interpreters.
