Application for Admission to the Clinical Laboratory Science (Medical Technology) Program (2009-2010)

Program Description:
The Clinical Laboratory Science (Medical Technology) Program is offered in Boise/Meridian and Pocatello, Idaho. To be considered for priority admission, your complete application with all required materials must be received by May 15, 2009.

Prior to applying, all prerequisites must be complete or in-progress with anticipated completion dates prior to the start of the Fall 2009 classes.  Acceptance into the program will be conditional upon successful completion of prerequisites.  The essentials of the program must be read (click here - NAACLS essentials).  Submission of your application implies agreement and you understand that a signed copy will be required to be kept in your student file.  Acceptance into Idaho State University does not guarantee acceptance to the Clinical Laboratory Science Program.  After the priority admission deadline of May 15, 2009, CLS applicants may still be accepted if space is available.

Application Process:
CLS Student applicants must be admitted to Idaho State University, complete the online CLS application form, write a letter of application (to explain why CLS and your career goals), arrange for three references to submit recommendation forms, submit copy of transcripts of all colleges and universities attended (other than ISU).  Applications will not be reviewed until all application materials have been received.

  • Non-ISU students will need to apply for admission to ISU (click here)
  • Applicants should submit a  completed application form online by May 15.
  • Applicants should write a letter of application and submit either by mailing to address indicated below or submit online by May 15. (Submit here)
  • Arrange for three CLS reference recommendation forms to be completed and submit either by mailing to address indicated below or online by May 15.  (Please submit reference recommendation forms here)
  • Send a copy of their official academic transcripts (other than ISU) to:

Admission to the CLS Program is competitive.
  • If there are more qualified applicants for entry than clinical experience sites and the program can handle, selection will be made using the student evaluation rank system. (Click here for further information)
  • Qualified Students not placed into clinical experience sites during the summer semester, may qualify for placement during the following fall and spring semesters.
  • Successful candidates will be notified prior to the beginning of the fall semester.
  • Those students selected for the program must meet with one of the CLS/MT faculty for advisement on course registration.
  • The number of students accepted into the CLS Program is dependent on the clinical experience sites available and university resources.
  • Course prerequisites are published in the ISU course catalog (click here).  ALL prerequisites must be completed prior to start of program, unless written prior permission is given by CLS Program Director.
