Take the Personal Health Assessment (also known as the Health & Well-being Assessment)! It’s a confidential health questionnaire you may complete online providing you with a free customized individual report. Annually, Health Matters will remind you to complete an Assessment. It’s a great opportunity for you to get an overview of your health habits – what you’re doing that is health wise and what you can do differently. The areas covered in the assessment include medical, health, pain, sleep, body weight, nutrition, physical activity, stress, lifeload, risk behavior, perception of general health, perception of effectiveness, job satisfaction, and mood.
This is a voluntary assessment available to all State of Idaho employees; it is accessed through the Blue Cross Website, and provided by a company called VieLife who protects and stores the data. Individual participant information is not shared with anyone, including your employer. However, Health Matters will receive general information regarding where education efforts could be focused.
To be eligible to win a free bodybugg from BodyMedia and Apex Fitness, you must be a State of Idaho employee and you must complete your survey online between October 19, 2009 and December 18, 2009. You will automatically be entered to win the bodybugg.
Take the Assessment this year, even if you did not take it last year. If you took the Assessment last year, just use your password and user id to logon to the Blue Cross website to access the Assessment. If you can’t locate your logon information, do an online inquiry on the Blue Cross Home Page. Having trouble – contact Blue Cross Customer Service at 1-866-804-2253 or 208-331-8897 and they will assist you.
