  The headwaters
of the Portneuf River are in northern Caribou County, very close to Bingham County in southeast Idaho. The river flows south in Caribou County and into Bannock County north of Lava Hot Springs. At Lava Hot Springs the river turns west. Near McCammon the Portneuf turns north and flows north and west to American Falls Reservoir. North and west of Pocatello, the river forms the boundary between Bannock and Power Counties.
Both the headwaters and the confluence of the Portneuf with the Snake River are on the Shoshone-Bannock Reservation. Over the course of its flow, the Portneuf travels about 156 km (96 miles).
