Animal Care Facility Forms
Protocol Form in rich text formats. All uses of animals in research, field, or teaching labs must fill out the following form. There are special sections to be filled out for fieldwork and teaching labs.
All Graduate Teaching Assistants, Graduate Research Assistants, and others students must have Personnel Statements, Occupational Health and Safety form, and completed online CITI course to be able to work with animals. Please contact Facility Manager for access to the Animal Care Facility.
