Obtaining Medical Documentation

The following list of documentation guidelines are provided to assist students in obtaining appropriate documentation from qualified medical professionals. The information provided about your disability is only one part of determining necessary accommodations. The provision of appropriate documentation to an institution helps students educate staff and faculty about the impact of their disabilities on their current functioning, and the probable need for accommodations.
