Due Dates: December 1 for subsequent Fall Semester, July 1 for subsequent Spring Semester, and August 1 for subsequent Summer Semester (Note: Submit prior to these dates to allow time for feedback and processing).

Periodic Performance Review (PPR) Form (updated 11-2012)

Post-Semester Faculty Activity Report Form (updated 11-2012). Faculty are required to complete a Post Semester Faculty Activity Report Form at the end of each semester (including summer for 12-month faculty).  Each faculty member (.5 FTE and higher) will complete the form.  The University Workload Policy mandates that each college, or major division within a college, develop metrics for assignment of WEUs to instruction, research/creative activities and professional service.  It is this weighting system that determines the WEUs associated with each course taught and other faculty activities.

Academic Affairs Budget Request Form - All requests for funds to the Provost and Vice President for Academic Affairs will need to be submitted on this form.  All decisions will be documented and returned to the college.  If approved, spending will not be allowed until the actual transfer of funds has has been processed and is visible in Banner.  If the form is submitted electronically, please forward to acadvp@isu.edu.
