Paula Phelps Paula Phelps was promoted to Director of the Department of Physician Assistant Studies in 2010. Paula graduated from the University of Utah Physician Assistant Program in 1995 and completed her Master's in Health Education at Idaho State University in 2000. Her clinical experience is in family practice & OB/GYN. Mrs. Phelps was hired by the ISU PA Program in 1995.
 
Sherwin D'Souza, MD

Sherwin D'Souza Dr. Sherwin D'Souza is a graduate of GS Medical College in Mumbai India. He completed his internship at St. Lukes Roosevelt in New York City and went on to complete his residency and Chief residency in Internal Medicine at the University of Connecticut Health Sciences Center. He has served as Medical Director and a preceptor for the PA program since 2005.
He has practiced in Idaho since 2000 and is currently in private practice.His practice specializes in diabetes, osteoporosis and other endocrine conditions.
He is also a Clinical Instructor at the ISU Family Practice residency as well as adjunct Faculty at the University of Washington and Volunteer Clinical Instructor at the University of Utah.
 

Michael A. Patmas, MD

Sherwin D'SouzaMichael A. Patmas, MD, FACP is a board certified internist, Fellow of the American College of Physicians and the American College of Physician Executives.. He obtained his MD degree from the University of Nevada School of Medicine where he also completed an internal medicine residency. He began practice as a clincial assistant professor of medicine at Nevada. He then went into private practice before returning to academics at the University of Pennsylvania Health System. In 2000, he became the Medical Director of Ambulatory Care and Education at Providence Portland Medical Center in Oregon. He obtained a master's degree in management from the University of Southern Calfornia - Marshall School of Business in 2002 becoming a Certified Physician Executive. In 2005 he became medical director of the Central Oregon IPA and Vice-President of Clear Choice Health Plans in Bend, Oregon. In 2007, he came to Idaho as Chief Executive Officer at St. Alphonsus Medical Group. During 2009 he practiced internal medicine in Eagle, Idaho and also served as Regional Medical Director for Idaho - Correctional Medical Services. He joined ISU in 2010.
 
Ann Smalley, MBA

Ann Smalley Ann completed her Master's of Business Administration at Idaho State University in 1985. She manages the program finances and assists both the academic & clinical administrative staff when needed. Ann is the program's administrative assistant to the program director. She is also a Notary. Ann was hired by the ISU PA Program in 2003.

Kent Whitaker Kent graduated from the University of Utah Physician Assistant Program in 1995 and completed his Masters of Education at Weber State University. His clinical experience is in primary care and dermatology. Mr. Whitaker was hired by the ISU PA Program in 1998.

Dave Martin Dave graduated from the University of Oklahoma Physician Assistant Program in 1980 and completed his Master's in Physician Assistant Studies at the University of Nebraska in 2000. His clinical experience is in family practice & emergency medicine. Mr. Martin was hired by the ISU PA Program in 2003.

Lisa SalazarLisa holds Master's Degrees in Public Health and Educational Technology from Idaho State University. In addition, she is currently completing her PhD in Instructional Design. Lisa's areas of interest and research include improving quality of patient education (including health literacy and cultural competency), personal wellness, and occupational/environmental health. She teaches research and evidence-based medicine and works with the faculty and program staff to coordinate research and perform systematic program evaluation.

David TalfordDavid earned his Master's degree in Physician Assistant Studies through University of Nebraska Medical Center, and has been practicing clinically since 1999. David has practice experience in urgent care, family practice, and dermatology. David was hired by the ISU PA Program in 2007.

Corbin Bunnage Corbin graduated from Idaho State University Physician Assistant Program in 1999. His clinical experience is in emergency medicine. He received his MS degree with an emphasis in family practice and emergency medicine from the Arizona School of Health Sciences in March 2004. Corbin was hired by the ISU PA Program in 2003.

Jared PapaJared is experienced in rural and underserved community health having worked in the fields of family and urgent care medicine.  He is fluent in Spanish.  Jared received his Physician Assistant training from Pacific University, graduating in 2003, and joined the ISU PA Program in 2008.

Jeff Johnson Jeff graduated from the ISU PA Program in 2002. He is also a graduate of the ISU College of Pharmacy earning a PharmD degree. His experience is primarily in emergency and urgent care medicine. He has also worked as a community pharmacist . He joined the ISU PA Program in 2011.

Anntara SmithAnntara graduated from Idaho State University in 2003. She has been practicing in the Boise area since that time and clinical experience includes Family Medicine, HIV, and Psychiatry. She joined ISU's PA program in 2011.

Chad HarballChad has over 10 years of computer, telecommunications, and business operations experience, specializing in computer hardware and synchronous distributed education. He oversees classroom operations, inventory, building safety, medical & electronics purchasing, and logistics planning. Chad was hired by the ISU PA Program in 2006.

Vicki Allen Vicki has been a Registered Nurse since 1976, She became a Legal Nurse Consultant in 2005. Vicki's clinical experience is in neonatal and adult intensive care. She has worked as a staff nurse, manager and hospital administrator prior to being hired by the PA program in 2010.

Jeanne LancasterA former ISU student, Jeanne has worked for the University in several departments including, Student Services. She previously worked for the Southeast Idaho District Health Department. She transferred to the ISU PA Program in 2003.

Brian RedfieldBrian graduated from Brigham Young University in 1983 and has spent 25 years working nationally in commercial, government and educational television. Brian has been certified in telecommunications network design and implementation. Brian was hired by the ISU PA Program in 2007.
 

Jerry Jones

Jerry JonesJerry holds a B.A. in broadcasting. He worked as an anchor with Idaho's largest TV news team in the 80s and 90s before gaining additional experience as a member of the City of Boise community relations staff. Jerry also consulted for Ulead Systems, a global software developer, before joining the ISU PA program staff in 2007.

David Hachey Dr. Hachey is a Clinical Associate Professor at Idaho State University College of Pharmacy in the Departments of Pharmacy Practice and Administrative Science and Family Medicine. He earned his PharmD at the University of Rhode Island in 1998 and completed a residency and fellowship in family medicine at Idaho State University in 2000.

Holly Sorsch Holly holds a Bachelor of Business Administration degree in Marketing from Idaho State University and an Associate of Arts in Interior Design from Bellevue College. Before joining the ISU PA Program in 2011, Holly was living in Athens, Greece working with a social justice organization supporting victims of Human Trafficking.

Alan Mirly Alan graduated from Idaho State University Physician Assistant Program in 2002. His clinical experience is in Otolaryngology and Neurosurgery. He received his MBA with focus in Health Care Administration from the University of Phoenix in 2011. Alan was hired by the ISU PA program in 2012.

Jennifer Forbes Jennifer graduated from the University of South Alabama Physician Assistant Program in 2001 where she earned her Master of Health Sciences degree. She has clinical experience in pediatric orthopedics, pediatrics, and family practice. She served as an assistant professor in two other PA programs prior to joining the ISU PA program in 2012.
